If you haven’t figured out your research project yet, now is your chance to try one out.  You won’t have to stick with it if you change your mind later.

Note that class projects like this are exempt, but pretend this is for your thesis, and you plan to publish the results, where does your project fall in terms of oversight?

You do not need to generate a PDF or word file, just answer the questions below in regular text using the blog. You also do not need to re-copy the text from the form just the relevant headers for the sections that you need to fill out. Use the fields and headings below.

Provide an outline of all procedures to be followed. Attach copies of questionnaires and/or interview questions with this application, including non-English translations.

INFORMED CONSENT
Informed consent must be obtained before collecting data from subjects. Attach a copy of the consent form(s) to be used. The consent form must list contact information for the Principal Investigator and a representative of the RSO.
